Care Coordination

Acentra Health is working with the Arkansas Department of Human Services to provide Care Coordination services to spenddown category beneficiaries deemed eligible for Home and Community Based Services based on an Optum Independent Assessment. Participation in this program is free to the beneficiary and is designed to assist in meeting the beneficiary’s everyday healthcare needs.

The program helps manage the individual’s healthcare; including:

• Help understand and manage health conditions.
• Assist in talking with primary doctor and other providers.
• Refer to helpful resources in local community.
• Collaborate with individual and their healthcare providers to develop a comprehensive care plan tailored to specific needs and health goals.
