We are reaching out to inform you of a recent update to the authorization submission process for Inpatient Psychiatric Services and Non-waiver Personal Care Services. A brief questionnaire has been added to collect the most accurate and up-to-date contact information for beneficiaries and/or their legal guardians for the Optum Independent Assessment (IA) Referral Process.

This enhancement is designed to streamline and improve the referral process by ensuring we have the correct details to facilitate timely and effective communication to Optum.

We appreciate your cooperation in completing this questionnaire as part of each authorization submission. Your support helps us better serve beneficiaries and their families.
