Update
Effective September 1, 2026, Acentra Health reviewers will review only those procedure codes submitted on the authorization request clinical lines. Providers are responsible for entering all requested procedure codes and associated tooth numbers on the request. Services referenced in notes or messages, but not included on the clinical lines, will not be reviewed or considered for authorization.
To avoid processing delays, please ensure that all requested services are accurately entered at the time of submission.
